This form is reserved for formal ethics complaints against addiction or mental health professionals certified through ACBHC. Please read the information below carefully before submitting.

Confidential & Secure Submission All information submitted through this form is handled confidentially by the ACBHC Ethics Committee. Your submission will be reviewed in accordance with the established Procedures for Adjudicating Ethics Complaints.

How Complaints Are Processed

Complete This Form
Provide your information (Complainant), the respondent's information, and any witnesses who have direct knowledge of the alleged conduct.

Identify the Ethical Violations
List each Ethical Principle and Section you believe was violated, with a brief description of the behavior in question.

Attach Supporting Evidence
Include any documentation that supports your allegations.

Sign & Submit
Your electronic signature authorizes ACBHC to release the information necessary to investigate this complaint. The complaint will not be processed without it.

Logged & Acknowledged
Your complaint is date-stamped and recorded in the Ethics Complaint Log. Both you and the respondent are notified within 10 days that it's been received.

Investigate or Dismiss
The Ethics Review Committee decides within 10 days whether your complaint warrants investigation or should be dismissed, and notifies both parties of that decision within 10 days.

Investigation
If investigated, review is typically completed before the next scheduled Commission meeting, with up to a 90-day extension if more time is needed. In serious cases, the Commission may vote to temporarily suspend the respondent's certification while the investigation is pending.

Hearing & Determination
Once the investigation is complete, the Ethics Review Committee holds a hearing with the full Commission. You and the respondent (who may bring their own legal counsel) are both invited to give testimony before the Commission reaches a determination by vote in closed session.

Final Notification
The Commission's determination is sent by certified mail within 5 days. If a sanction is issued, the respondent is advised of their right to appeal the decision.
